It looks like you've got a pretty strong grip. Makes it a little more challenging to swivel and roll. Also why your clubface is facing the sky at the end of your swing with a flat left wrist. Try taking your grip at impact fix before startup. See if it weakens a bit. Then hit little 175 yard drives with a full roll follow-through and get use to the rhythm of horizontal hinging. However - quick fixes over the internet don't hold a candle to a live, 3 dimensional lesson with one of our instructors.
