I still think there's an OTT tendency in your swing. Not with regards to plane and external references, but with regards to your shoulder turn.
You're dipping your right shoulder through impact - pretty much like Paula Creamer. She gets away with it (at least she did last week), but it is not the best way. You should be dipping it during transition instead. Hip forward & right shoulder down. The hands will follow. Then turn through.
If dipping is problematic for you, you need a flatter swing plane.
You have a keen eye, but you didn't reach the root of the problem.
Me and Rob Noel, taught by Yoda and GSEM in Abita Springs, LA, actually figured it out.
At the beginning of start down my entire body slides towards the ball a few inches. We had been struggling with my pivot for weeks and I told him that I had noticed that in just the past couple of days in my garage.
He then had me hitting with my butt against a butt board (basically keep your butt attached throughout the swing) and all of the sudden I had acres of space for my hands and shoulders to move down.
